Anyone Know a Good Tooth Whitener?

I'd like to whiten my teeth at home as I've recently noticed they aren't looking exactly... sparkly! I suspect my former years of being a smoker and latter years of being a coffee drinker may be catching up with me! Does anyone have any experience of tooth whitening kits and any good ones?
